How to Become an LMHC in Massachusetts

Massachusetts accepts exactly one licensing exam for mental health counselors: the National Clinical Mental Health Counseling Examination (NCMHCE), administered by the NBCC. The Board of Registration of Allied Mental Health and Human Services Professions also requires a master’s built on at least 60 semester credits and 3,360 hours of post-master’s experience, completed in no less than two and no more than eight years. The board revised these rules on July 31, 2026.

Which exam does Massachusetts accept for the LMHC?
Only the NCMHCE. The board’s regulation defines its Board Examination as the National Clinical Mental Health Counseling Examination administered by the National Board for Certified Counselors, and its application guide states that no other exam will be accepted. A passing score stays valid for five years from the date you took it.
How many hours does Massachusetts require?
3,360 post-master’s hours in total, including 960 hours of direct client contact — at least 610 of them in individual, couples or family counseling, and no more than 350 in group counseling. You also need at least 130 hours of supervision, 75 of them individual, with at least one supervision hour for every 16 hours of direct client contact. The experience must take between two and eight years.
What degree does Massachusetts require?
A master’s in mental health counseling or a relevant field, from a program of at least 60 semester credits (or 80 quarter credits), including graduate courses across the ten content areas the regulation lists. The regulation does not make CACREP accreditation a requirement.
What does an LMHC licence cost in Massachusetts?
A $117 non-refundable application fee plus a $155 licence fee, $272 in total, with a possible processing fee depending on how you pay. Renewal is $155 every two years. Applications are accepted only online, through the Health Professions Licensing Portal. The NCMHCE fee itself is paid to NBCC.
What is the new Licensed Supervised Mental Health Counselor licence?
The July 31, 2026 revision created a second licence, the Licensed Supervised Mental Health Counselor (LSMHC). Its applicants must also provide a passing NCMHCE score, and it carries the same $117 application and $155 licence fees.

Question 1

Maya has made clear she is 'only here because Mom's forcing me.' In this first meeting, what is the best way to begin building an alliance with her?

▸ Why B is the answer

Naming her reluctance out loud and handing her some control is the heart of joining a teen who was made to come. You acknowledge she did not choose this, promise not to push, and ask what she would want, which turns coercion into a small genuine choice. That is what earns the trust of an involuntary adolescent client.

Question 2

Maya says the anger 'comes out of nowhere' and she 'can't stop it once it starts,' then looks down, ashamed. Which response best serves both the alliance and the work right now?

▸ Why C is the answer

Naming the fear and shame she is carrying, and then getting curious together about what comes just before the anger, does two things at once. It tells her she is understood at the moment she feels most exposed, and it quietly begins mapping the pattern that regulation work will later build on. You meet the feeling first and let understanding, not fixing, lead.

Question 3

Maya says that during the worst moments she sometimes thinks 'it would be easier if I just wasn't around,' but adds she would never act on it, has no plan, and the thought passes when she calms down. How do you handle risk here?

▸ Why D is the answer

Passive thoughts of not being around, in a distressed and dysregulated teen, warrant a direct but calm assessment: how often, how intense, any intent, any plan, any access to means, and what helps them pass. You document it and put a simple safety plan in place. That is thorough without over-reacting, and it treats her disclosure with the seriousness it deserves.
