NCMHCE Exam Format & Scoring

Before you study, it helps to know exactly what you're walking into. Here's how the NCMHCE is built and scored, in plain terms — followed by free practice cases in the real format.

Written and reviewed by the LexPort exam-prep team, led by our Founder & Exam-Prep Lead. LexPort builds certification practice questions — this is exam-preparation material, not medical advice. Last reviewed July 2026.

How many case studies are on the NCMHCE?
The current form has 11 clinical case studies. Each one unfolds as a simulation where you make a series of counseling decisions.
How long is the exam?
You have 255 minutes. That's a long sitting, which is why building timed stamina with full-length mock exams matters as much as knowing the content.
How many questions are scored?
About 100 items are scored, out of roughly 130–150 total. The remaining items are unscored pretest questions being trialed for future exams — you can't tell which is which, so treat every item as if it counts.
How is it scored and what's the cut score?
NBCC sets the passing standard for each form through a standard-setting process rather than publishing a single fixed percentage. The practical takeaway: aim for consistent, correct clinical reasoning across all five content areas.
What content areas does it cover?
Five: Counseling Skills & Interventions (~30%), Intake, Assessment & Diagnosis (~25%), Professional Practice & Ethics (~15%), Treatment Planning (~15%), and Core Counseling Attributes (~15%).

What the questions actually look like

The format is one thing; the questions are another. Here are three realistic examples. The full simulator has 4,000+, timed and scored.

Question 1

Given your impression that this may be delirium, what is the most appropriate step regarding Eleanor's care today?

▸ Why A is the answer

Suspected delirium is a medical emergency. The underlying cause, here likely an infection and a medication effect, can be dangerous and is frequently reversible when caught early, so the priority is a same-day medical evaluation, not a counseling plan. Recognizing that this belongs in medicine today, and coordinating it, is the single most important move a counselor makes in this situation.

Question 2

Eleanor lives alone. Alongside arranging medical evaluation, how do you best address her safety right now?

▸ Why B is the answer

Two kinds of risk are live at once. The passive suicidal remark must be assessed directly for intent, plan, and means, especially given her fluctuating clarity. And delirium itself makes a confused elder living alone physically unsafe right now, through falls, wandering, mismanaging medications, or leaving the stove on. Ensuring she is not alone until she is evaluated is core safety, and it holds both dangers together rather than choosing one.

Question 3

You want to explain a plan and obtain consent, but Eleanor drifts in and out of tracking the conversation. How do you handle informed consent and her capacity to decide?

▸ Why C is the answer

Capacity is decision-specific and, in delirium, genuinely changeable through the day. The sound approach is to assess her ability to understand this particular decision, use her clearer windows to explain it and seek her own assent, bring Ruth in with Eleanor's permission, and document that her capacity is fluctuating. That protects her rights and her dignity without either stripping her of a voice or pretending nothing is wrong.
