How to Pass the NCMHCE

The NCMHCE is a clinical-simulation exam: you work through case studies and make the decisions a competent counselor would make. Passing it is less about memorizing and more about practicing the judgment the cases test. Here is an honest, practical plan — with real practice cases you can start right now.

Written and reviewed by the LexPort exam-prep team, led by our Founder & Exam-Prep Lead. LexPort builds certification practice questions — this is exam-preparation material, not medical advice. Last reviewed July 2026.

How is the NCMHCE structured?
It presents clinical case studies — 11 on the current form — and asks you to make assessment, diagnosis, treatment-planning, intervention, and ethical decisions within each one. You have 255 minutes. Roughly 100 items are scored across about 130–150 total, with the rest unscored pretest items.
What score do I need to pass?
NBCC sets the passing standard per exam form through a formal standard-setting process, so there is no single fixed percentage published. Practically, aim to reason correctly through cases consistently rather than chasing a specific number.
How should I spend my study time?
Spend most of it on case-based practice, not passive review. The exam only tests skills that appear inside a scenario, so working full cases — deciding, then reading why each option wins or loses — builds the exact reasoning that's scored. Weight your practice toward the two biggest areas: Counseling Skills & Interventions (~30%) and Assessment & Diagnosis (~25%).
A simple week-by-week plan?
Weeks 1–2: one full case a day, untimed, reading every rationale. Weeks 3–4: focus on your weakest domain and on risk/ethics decisions. Final 1–2 weeks: full-length timed mock exams to build stamina for 255 minutes, then review every missed item. Adjust to the time you have.
What's the most common reason people fail?
Choosing the technically-correct-but-mistimed answer, and mis-prioritizing safety and ethics. Both are fixable with case practice that forces you to justify the order of your decisions.

Start by finding your gaps

Every honest study plan starts with knowing what you don't know. Try three practice questions. The full simulator has 4,000+, timed and scored.

Question 1

With Hana now reporting shaky, sweaty mornings that a drink relieves, what is the most appropriate level-of-care step at this point?

▸ Why A is the answer

Morning symptoms relieved by alcohol point to physiological withdrawal, which is a medical question that outpatient counseling alone cannot answer or safely manage. A medical evaluation determines whether she needs a supervised taper or medically managed detox, and counseling continues in parallel. Matching care to her medical risk is the level-of-care decision here.

Question 2

Hana is discouraged after the heavy night but has five real weeks behind her. Consistent with the spirit of motivational interviewing, how do you best strengthen her motivation now?

▸ Why C is the answer

Motivational interviewing grows motivation from the client's own words, so you evoke her account of how she strung together five weeks. Hearing herself describe what worked builds self-efficacy, the belief that she can do this, and having her name the next step keeps it hers. That is the spirit of the approach: draw change out of her rather than supply it.

Question 3

Eleanor moves between missing George, her poor sleep, her weight loss, and feeling 'foggy,' and it is hard to tell what is driving her distress most. What is the most useful way to assess her presenting problem and level of distress right now?

▸ Why B is the answer

A good grief intake anchors the distress to the loss and its timeline. Asking her to tell the story of George's death and how her days changed since organizes a scattered picture, shows whether her symptoms track the loss, and gathers presenting problem, onset, severity, and impairment at once. It also honors the loss, which builds the alliance.
